Many insurance plans require a referral from your primary care physician. However, we accept self-referrals and can often help you navigate the process. Call our office and we'll check your specific plan.
We work hard to offer same-week appointments for urgent concerns. Routine new patient visits are typically available within 1–2 weeks. Telehealth appointments often have the shortest wait times.
Please bring a photo ID, your insurance card, a list of current medications, any relevant lab work from the past 6 months, and completed intake forms (sent to you by email after booking).
Telehealth is available for most follow-up visits and many initial consultations. Certain procedures and physical exams require in-person visits. Your care team will advise you on the best option for your situation.
Yes. We prescribe and help manage all major CGM systems including Dexcom and FreeStyle Libre. We also assist with insulin pump therapy and offer diabetes technology clinics.
